Moisturising

Dry skin is more likely to break down and is more easily injured from friction1. It should be managed with moisturisers such as:

  1. Check skin. Document any changes (redness, cracks, fissures).
  2. Apply Sensi-Care™ Emollient Cream to dry, cracked, or chafed skin. Use small amounts and massage gently and thoroughly into skin.
    • Do not massage stage 1 pressure ulcers (nonblanching hyperaemia). This avoids additional trauma to compromised skin.
    • Do not apply between toes or in skin folds, where moisture may get trapped and cause maceration.
  3. Educate resident/carer on the importance of fluid intake and the need to reapply a skin conditioner/moisturising cream daily.
  4. Discard disposable items in a plastic bag and secure.
  5. Document findings, and update care plan.
